why??????

Josip Simunic was born in Australia and has an Aussie accent. When he was first cautioned, poor Graham showed him the yellow which he argued with his Aussie accent and it so happened in that recording the caution he put # 3(Simunic) under the Australia number 3 instead of the Croatia number 3. So when the time was to send him off, he say no Caution in his notebook for number three so he assumed just another caution.

Simple, costly mistake. Poor lad was on his way to refereeing the final however that mistake is inexcusable.

surely the fact he didn't have a bright yellow jersey on might have given it away. ha never mind

Well mate. If I were refereeing England vs Ghana and I caution a Ghanaian with an English accent. Dont ya think somwhere in ya mindd would have you think its an english player you just cautioned?

No, not really.

But pressure makes you do some strange things. Simple error of noting something down incorrectly when in a rush.
